Verify hazmat endorsement on the CDL

Confirm hazmat training within the last 3 years

Identify whether the load requires tank endorsement

Check the medical card and DQ file currency

Review the shipper-prepared shipping paper

Verify the 24-hour emergency response phone number

Determine whether the load is placardable

Confirm the shipper's certification signature

Apply placards to all four sides of the trailer

Photograph each placarded side

Mark UN identification numbers where required

Inspect packagings for leaks, dents, and damage

Segregate incompatible materials per 49 CFR 177.848

Block and brace cargo per 49 CFR 393 Subpart I

Verify the load did not exceed quantity limits

Plan the route around tunnel and bridge restrictions

Confirm the post-accident hazmat kit is on board

Check fire extinguisher charge and inspection tag

Hold the shipping paper within reach of the driver

Record whether an incident occurred in transit

Call CHEMTREC and the safety director from the scene

File the DOT Hazardous Materials Incident Report

Obtain consignee signature on the shipping paper

Remove or cover placards after the trailer is empty and purged

File shipping papers in the hazmat retention system
